Science behind the use of Combination of PRP and Micro Needling for
Alopecia:
Microneedling
Microneedling uses tiny needles to create controlled punctures in the scalp, triggering the body's natural wound-healing response. This leads to stimulation of stem cells and inducing activation of growth factors. With microneedling, there is a better absorption of topically applied adjunctive medicine, such as RPR in this case.
Additionally, wounding of the tissue with microneedling creates the cascade and migration of additional growth factors and stem cells leading to their increased tissue expression, particularly hair follicle growth factors and stem cells, which is the specific desired result in this case.

PRP (Platelet Rich plasma)
Uses injections of a patient's own concentrated blood platelets to accelerate healing and tissue growth. It supplies a concentrated dose of platelets rich in growth factors directly to weakened or dormant hair follicles, jumpstarting the regeneration process.
Platelets contain growth factors that are complex proteins that initiate and organize growing tissue by forming a fibrin stand (mesh) or extra cellular matrix under skin. Fibrin, Fibronectin, and Vitronectin.
PRP contains stem cells that are concentrated which allows for cell division for growth as well as being able to attract additional stem cells to the repair site. Stem cells then differentiate from each other, and tissue formation occurs.
Once these proteins and growth factors bind to cellular receptors, they activate intercellular events which help meditate:
-Angiogenesis (formation/growth of new blood vessels)
-New cell proliferation
-Migration of stem cells to area
-Production of extracellular matrix proteins (increased collagen production)

Important Notes regarding this Procedure:

• Patients should be advised to use a gentle, daily hair shampoo and conditioner.
Patients using minoxidil or finasteride may resume using these products at (3)
weeks.
• The underlying Testosterone/DHT conversion in men usually does not yield great results since the underlying issues remain untreated. In women, hair loss is typically
endocrine system related and yields more dramatic results in reversing hair loss
• PRP as a stand-alone treatment for males usually does not have the same results as in women. However, PRP is very popular as part of combination therapies such as using in conjunction with prescription medications, red LED lights, and transplantation procedures (greater uptake of the grafts).
